Alternatively, individuals may submit a registration form and provide payment by check. Please enter your registration information, print the form, and mail or fax to the PDP office.
If an employer is providing payment to cover the tuition cost, please either register online or submit a registration form indicating "Employer providing payment" in advance of the registration deadline. Due to space limitations we cannot reserve a place unless the tuition has been paid-in-full.
Registration Policies
A $25.00 late fee will be applied to registrations received after the deadline. On-site registration is limited and based upon availability. Please register early.
PDP accepts payment in the form of check, American Express, Discover, MasterCard, or Visa. Make checks payable in US dollars to "SSA, University of Chicago" and mail to our office. If paying by credit card, please visit our online registration page.
Complete payment must accompany all registrations. You will receive confirmation of your registration via email. If you have not received an email confirmation of your registration within 48 business hours, please contact the PDP office. Persons with a disability who may require special assistance should call 773.702.1166 well in advance to the start of the class.
