Alternatively, individuals may submit a registration form and provide payment by check. Please download the form, open in Adobe Acrobat, and enter your registration information. You can print the form and mail or fax to the PDP office with payment. Or, submit the form by email and mail payment.
If an employer is providing payment to cover the tuition cost, please either register online or submit a registration form indicating "Employer providing payment" in advance of the registration deadline. Due to space limitations we cannot reserve a place unless the tuition has been paid-in-full.
A $25.00 late fee will be applied to registrations received after the deadline. On-site registration is limited and based upon availability. Please register early.
PDP accepts payment in the form of check, American Express, Discover, MasterCard, or Visa. Make checks payable in US dollars to "SSA, University of Chicago" and mail to our office. Credit card payments must be made through our online registration page.
Complete payment must accompany all registrations. You will receive confirmation of your registration via email. If you have not received an email confirmation of your registration within 48 business hours, please contact the PDP office.

Persons with disabilities who need an accommodation in order to participate in a program should contact the PDP office at least one week in advance of the event for assistance.
